GS-12 Clinical Pharmacist is a licensed professional with knowledge and proficiency in pharmacy practice, including pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics.Clinical Pharmacist duties include but are not limited to the following:
Performs continuous evaluation of prescribed medications to assure optimal drug therapy by analyzing real and potential drug-related medical problems and implementing corrective action to ensure that patients receive optimal drug therapy.
Provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics;monitoring for efficacy, side effects and clinical outcome;advises prescribers as appropriate.
Provides patient-specific therapeutic drug monitoring and communicates relevant findings/recommendations to other health care providers in charge of the patient both orally and in writing.
Reviews charts, evaluates pertinent clinical and pharmacokinetic laboratory data to evaluate the efficacy of drug therapy, drug-drug and drug-nutrient interactions, monitors for adverse drug effects, and screens for allergies.
Reviews chart and all medication orders for therapeutic duplication in the patient's drug regimen
Reviews all medication orders for appropriateness, drug selection, dosage, route of administration and the amount, prior to dispensing.
Provides medication counseling to patients;completing medication histories including patient interviews as appropriate.
Serves as a drug information resource by providing up-to-date drug information as necessary to prescribers, other health care professionals, patients and caregivers.
Ensures drugs dispensed are completely and correctly labeled and packaged in full compliance with Federal rules and regulations.
Identifies, interprets and resolves prescribing and dispensing issues, including pharmacy therapeutics and the formulary.
Work Schedule: Part-Time and Full-Time positions available.Hours are varied.Nights, weekends, and holidays will be required.All positions have the possibility of rotating shifts to suit the needs of the Service Line
